Manager, Care Management (Maternal-Child Health)

1. Develop and implement care management strategies and initiatives to improve the health outcomes of mothers and children enrolled in Humana’s health plans.
2. Lead a team of care management professionals, providing guidance, support, and training to ensure high-quality and efficient care management services.
3. Collaborate with cross-functional teams, including medical directors, providers, and community resources, to develop comprehensive care plans for members.
4. Monitor and analyze data to identify trends and areas for improvement in maternal-child health outcomes, and use this information to inform care management strategies.
5. Stay up-to-date with the latest research and best practices in maternal-child health to ensure the delivery of evidence-based care management services.
6. Develop and maintain relationships with community organizations and resources that support the health and well-being of mothers and children.
7. Ensure compliance with all applicable regulations and guidelines related to maternal-child health and care management.
8. Act as a point of contact for escalated cases and provide guidance and support to care management staff in handling complex cases.
9. Conduct regular performance evaluations and provide ongoing coaching and development opportunities for care management staff.
10. Collaborate with other managers and leaders within the organization to identify opportunities for process improvement and implement changes to optimize care management services.
11. Represent Humana in meetings and events related to maternal-child health, building relationships with key stakeholders and promoting the company’s mission and values.
12. Contribute to the development and implementation of company-wide policies and procedures related to maternal-child health and care management.
13. Participate in budget planning and management for the care management department, ensuring efficient use of resources and adherence to financial goals.
14. Foster a positive and inclusive work environment that promotes teamwork, innovation, and continuous learning.
