
Manager, Care Management (Maternal-Child Health)

- Develop and implement care management strategies and initiatives to improve the health outcomes of mothers and children enrolled in Humana’s health plans.
- Lead a team of care management professionals, providing guidance, support, and training to ensure high-quality and efficient care management services.
- Collaborate with cross-functional teams, including medical directors, providers, and community resources, to develop comprehensive care plans for members.
- Monitor and analyze data to identify trends and areas for improvement in maternal-child health outcomes, and use this information to inform care management strategies.
- Stay up-to-date with the latest research and best practices in maternal-child health to ensure the delivery of evidence-based care management services.
- Develop and maintain relationships with community organizations and resources that support the health and well-being of mothers and children.
- Ensure compliance with all applicable regulations and guidelines related to maternal-child health and care management.
- Act as a point of contact for escalated cases and provide guidance and support to care management staff in handling complex cases.
- Conduct regular performance evaluations and provide ongoing coaching and development opportunities for care management staff.
- Collaborate with other managers and leaders within the organization to identify opportunities for process improvement and implement changes to optimize care management services.
- Represent Humana in meetings and events related to maternal-child health, building relationships with key stakeholders and promoting the company’s mission and values.
- Contribute to the development and implementation of company-wide policies and procedures related to maternal-child health and care management.
- Participate in budget planning and management for the care management department, ensuring efficient use of resources and adherence to financial goals.
- Foster a positive and inclusive work environment that promotes teamwork, innovation, and continuous learning.
Bachelor's Or Master's Degree In Nursing, Health Administration, Or A Related Field.
Minimum Of 5 Years Of Experience In Care Management, Preferably In Maternal-Child Health.
Strong Leadership Skills And Experience Managing A Team Of Care Managers.
Knowledge Of Health Care Regulations And Policies Related To Maternal-Child Health.
Excellent Communication And Collaboration Skills With The Ability To Work Effectively With Interdisciplinary Teams, Providers, And Community Resources.
