- Lead and manage a team of care managers to ensure the delivery of high-quality care to Humana members.
- Oversee the coordination of care for members, including conducting assessments, developing care plans, and monitoring progress.
- Utilize clinical knowledge and experience to support and guide the team in making evidence-based care decisions.
- Develop and implement strategies to improve the overall quality and effectiveness of care management services.
- Monitor and track team performance metrics, providing feedback and coaching to ensure individual and team goals are met.
- Collaborate with other departments and external partners to ensure seamless and efficient care for members.
- Stay up-to-date with industry trends and best practices in care management to continuously improve processes and outcomes.
- Act as a liaison between the team and upper management, communicating updates, challenges, and successes.

Bachelor's Or Master's Degree In Nursing, Social Work, Or Related Field
Minimum Of 3-5 Years Experience In Care/Case Management, Preferably In A Healthcare Setting
Knowledge Of Managed Care Principles, Regulations, And Best Practices
Strong Leadership Skills, With The Ability To Effectively Manage And Motivate A Team
Excellent Communication And Interpersonal Skills, With The Ability To Collaborate With Various Stakeholders And Build Relationships With Members, Providers, And Other Team Members.
