  1. Oversee and manage a team of care/case managers, providing guidance and support to ensure the delivery of high-quality care to Humana members.
  2. Develop and implement care management strategies and processes to improve the health and well-being of our members.
  3. Collaborate with healthcare providers, community resources, and other stakeholders to coordinate and manage the care of our members.
  4. Monitor and evaluate the effectiveness of care management programs and make necessary adjustments to improve outcomes.
  5. Ensure compliance with all relevant regulations and guidelines, including HIPAA, CMS, and state-specific requirements.
  6. Develop and maintain relationships with key stakeholders, including members, families, providers, and community partners.
  7. Train and mentor care/case managers on best practices, policies, and procedures.
  8. Analyze data and trends to identify opportunities for improvement and develop action plans to address them.
  9. Act as a liaison between Humana and external organizations, representing the company and promoting our mission and values.
  10. Stay up-to-date on industry developments, trends, and best practices in care management to continuously improve our services.

Job Qualifications
  • Knowledge Of Medicare And Medicaid Regulations And Guidelines

  • Bachelor's Or Master's Degree In Nursing, Social Work, Or Related Field

  • Minimum Of 3-5 Years Experience In Care/Case Management, Preferably In A Healthcare Setting

  • Strong Leadership And Organizational Skills

  • Excellent Communication And Interpersonal Skills To Effectively Collaborate With Interdisciplinary Teams And External Stakeholders
