  1. Provide compassionate and high-quality care to clients in their homes.
  2. Collaborate with other healthcare professionals to develop and implement care plans for clients.
  3. Administer medications and treatments as prescribed by physicians.
  4. Monitor clients' health status and report any changes or concerns to the appropriate healthcare team members.
  5. Educate clients and their families on health management and self-care techniques.
  6. Utilize critical thinking skills to assess, plan, and implement effective nursing interventions.
  7. Maintain accurate and up-to-date documentation of client care.
  8. Adhere to all HIPAA and privacy regulations.
  9. Maintain a safe and clean environment for clients.
  10. Continuously strive to improve the quality of care provided to clients.
  11. Attend training and educational opportunities to stay current on industry trends and best practices.
  12. Maintain a positive and professional attitude while interacting with clients, their families, and other healthcare team members.
  13. Follow all Humana policies and procedures.
  14. Comply with all state and federal regulations related to home health care.
  15. Maintain a high level of integrity and ethical standards in all aspects of the job.

Job Qualifications
  • Current Lpn License: The Ideal Candidate For This Position Should Possess A Valid And Current Lpn License In The State Where They Will Be Working.

  • Home Health Experience: Humana Is Looking For An Lpn With At Least 1-2 Years Of Experience Working In A Home Health Setting. This Experience Should Include Providing Skilled Nursing Care, Medication Management, And Wound Care.

  • Strong Communication Skills: As This Position Involves Working With Patients In Their Homes, Excellent Communication Skills Are Essential. The Candidate Should Be Able To Effectively Communicate With Patients, Their Families, And Other Members Of The Healthcare Team.

  • Knowledge Of Medicare Guidelines: Humana Is A Medicare Provider, So Familiarity With Medicare Guidelines And Regulations Is Important For This Role. The Ideal Candidate Should Have Experience With Medicare Documentation And Billing Processes.

  • Ability To Work Independently: The Lpn In This Role Will Be Responsible For Managing Their Own Schedule And Providing Care To Patients In Their Homes. As Such, The Candidate Should Be Self-Motivated, Organized, And Able To Work Independently With Minimal Supervision.
