  3. Monitor and assess patients' health status and report any changes or concerns to the appropriate healthcare team members.
  4. Administer medications and treatments as prescribed by physicians.
  5. Educate patients and their families about their conditions, treatments, and self-care techniques.
  6. Maintain accurate and detailed patient records to ensure continuity of care.
  7. Follow all safety protocols and infection control measures.
  8. Communicate effectively with patients, families, and healthcare team members.
  9. Participate in team meetings and contribute to the development of patient care plans.
  10. Stay up-to-date with current nursing practices and standards.
  11. Adhere to all company policies and procedures.

Job Qualifications
  • Valid Lpn License: A Licensed Practical Nurse (Lpn) Is Required To Have A Valid And Current License In Order To Practice Nursing In The State Where The Job Is Located. This Ensures That The Individual Has Met The Necessary Education And Training Requirements To Provide Safe And Effective Patient Care.

  • Experience In Home Health Care: Humana Is Seeking Candidates Who Have Experience Working In Home Health Care Settings. This May Include Experience With Geriatric Or Pediatric Patients, Wound Care, Medication Management, And Other Skills That Are Commonly Utilized In Home Health Care.

  • Strong Communication Skills: As A Home Health Lpn, Effective Communication Is Key. The Ability To Communicate Clearly And Effectively With Patients, Their Families, And Other Members Of The Healthcare Team Is Essential For Providing High-Quality Care And Ensuring Patient Satisfaction.

  • Time Management And Organizational Skills: Home Health Care Often Involves Working Independently And Managing A Busy Schedule. The Ideal Candidate Will Have Strong Time Management And Organizational Skills To Ensure That All Patient Visits Are Completed In A Timely Manner And All Necessary Documentation Is Completed Accurately.

  • Knowledge Of Medicare And Medicaid Regulations: Humana Is A Leading Provider Of Medicare Advantage Plans, And Many Of Their Home Health Patients May Be Covered By Medicare Or Medicaid. Therefore, It Is Important For The Lpn To Have A Solid Understanding Of These Programs And Their Regulations To Ensure Compliance And Proper Documentation.
