Job Description

We are seeking a compassionate and dedicated individual with a current LPN license and at least one year of experience in a clinical setting.   1. Provide compassionate and high-quality care to our home health members.
  2. Utilize nursing skills and knowledge to assess, plan, implement, and evaluate patient care.
  3. Ensure all treatments and procedures are carried out correctly and in a timely manner.
  4. Maintain accurate and timely documentation of patient care.
  5. Collaborate with other healthcare professionals to develop and implement individualized care plans.
  6. Educate patients and their families on disease management, medication management, and other necessary health information.
  7. Monitor and report any changes in patient's condition to the appropriate healthcare team members.
  8. Adhere to all state and federal regulations, as well as company policies and procedures.
  9. Continuously seek opportunities for professional development and growth.
  10. Maintain a positive and professional attitude while providing care to patients.
  11. Act as an advocate for patients to ensure their needs are met.
  12. Maintain a safe and clean environment for patients.
  13. Communicate effectively with patients, families, and other healthcare team members.
  14. Participate in quality improvement activities to ensure the delivery of high-quality care.
  15. Adhere to ethical and legal standards of nursing practice.

Job Qualifications
  • Active Lpn License: Candidates Must Possess A Current And Valid Lpn License In The State Where They Will Be Providing Home Health Services. This Ensures That They Have Met The Necessary Educational And Training Requirements To Practice As An Lpn.

  • Prior Experience In Home Health Care: Humana May Require Candidates To Have Previous Experience Working As An Lpn In A Home Health Setting. This Demonstrates Their Familiarity With The Unique Challenges And Responsibilities Of Providing Care In A Patient's Home.

  • Knowledge Of Medicare And Medicaid Regulations: As Humana Is A Medicare And Medicaid Provider, Candidates Should Have A Thorough Understanding Of The Regulations And Guidelines Governing These Programs. This Includes Knowledge Of Documentation And Billing Requirements.

  • Excellent Communication And Interpersonal Skills: Lpns In A Home Health Setting Must Be Able To Effectively Communicate With Patients, Their Families, And Other Healthcare Professionals. This Includes Being Able To Provide Clear Instructions, Listen Actively, And Show Empathy And Compassion.

  • Strong Clinical Skills: Lpns Should Have A Solid Foundation Of Clinical Skills, Including Medication Administration, Wound Care, And Basic Patient Assessment. They Should Also Be Familiar With Common Chronic Conditions And Have The Ability To Recognize And Address Changes In A Patient's Condition.
