Home Health LVN/LPN - Pay Per Visit

- Provide compassionate and quality care to patients in the comfort of their own homes.
- Conduct regular home visits and assess patient's health status.
- Administer medications and treatments as prescribed by the physician.
- Monitor and document patient's progress and report any changes to the physician.
- Educate patients and their families on proper care and management of their health conditions.
- Collaborate with interdisciplinary team members to develop and implement individualized care plans.
- Maintain accurate and up-to-date patient records and documentation.
- Adhere to all state and federal regulations, as well as company policies and procedures.
- Provide emotional support and comfort to patients and their families.
- Maintain open communication with patients, families, and other healthcare professionals.
- Continuously improve knowledge and skills through ongoing training and education.
- Represent Humana in a professional and positive manner at all times.
- Adhere to ethical and legal standards of nursing practice.
- Ensure compliance with infection control and safety protocols.
- Participate in quality improvement initiatives to enhance patient care and outcomes.
Current Lvn/Lpn License In Good Standing: The Candidate Must Possess A Valid And Current License To Practice As An Lvn/Lpn In The State Where They Will Be Providing Services.
Minimum Of 1 Year Experience In Home Health: Humana Requires Candidates To Have At Least 1 Year Of Experience Working In A Home Health Setting, Preferably With Geriatric Patients.
Excellent Communication And Documentation Skills: As A Pay Per Visit Lvn/Lpn, The Candidate Must Be Able To Effectively Communicate With Patients, Their Families, And Other Healthcare Professionals. They Should Also Have Strong Documentation Skills To Accurately Record Patient Information.
Ability To Work Independently: Pay Per Visit Lvn/Lpns At Humana Are Expected To Be Self-Motivated And Able To Work Independently In A Home Setting. This Requires Strong Time Management And Organizational Skills.
Knowledge Of Medicare Guidelines And Regulations: Humana Is A Medicare Advantage Provider, So Candidates Should Have A Thorough Understanding Of Medicare Guidelines And Regulations, As Well As Experience With Medicare Billing And Documentation.
