  1. Develop and implement care coordination and coaching plans for Humana members in accordance with company policies and procedures.
  2. Collaborate with interdisciplinary team members to ensure the delivery of high-quality, patient-centered care.
  3. Conduct regular assessments of members' healthcare needs and identify potential barriers to care.
  4. Provide ongoing education and support to members and their families regarding their healthcare options and resources.
  5. Monitor and track members' progress and outcomes, making adjustments to care plans as needed.
  6. Act as a liaison between members, healthcare providers, and community resources to coordinate and facilitate access to necessary services.
  7. Maintain accurate and thorough documentation of all member interactions and interventions.
  8. Stay up-to-date on healthcare industry trends and best practices to continuously improve the care coordination process.
  9. Train and mentor new care coaches to ensure consistency and quality in care delivery.
  10. Adhere to all regulatory and compliance requirements related to member privacy and confidentiality.
  11. Participate in regular team meetings and contribute to the development and improvement of care coordination processes.
  12. Maintain a positive and professional attitude while representing Humana to members, providers, and the community.
  13. Continuously strive for excellence in customer service and member satisfaction.
  14. Handle member complaints and grievances in a timely and compassionate manner.
  15. Collaborate with other departments within Humana to ensure a seamless and coordinated approach to member care.
  16. Adhere to strict deadlines and prioritize tasks to meet the needs of members and the organization.
  17. Act as an advocate for members and ensure their voices and needs are heard and addressed.
  18. Proactively identify opportunities for improvement and propose solutions to enhance the overall member experience.
  19. Serve as a role model for other team members by promoting a positive and collaborative work environment.
  20. Continuously demonstrate a commitment to Humana's mission and values, as well as the well-being of its employees and members.

Job Qualifications
  • Excellent Communication And Interpersonal Skills

  • Bachelor's Degree In Healthcare Administration, Nursing, Or Related Field

  • Minimum Of 3 Years Experience In Care Coordination Or Case Management

  • Strong Knowledge Of Medicare And Medicaid Regulations

  • Prior Experience In Coaching Or Training Healthcare Professionals In A Clinical Setting
