  • Serve as a key member of the care navigation team, working closely with other healthcare professionals to provide comprehensive and coordinated care to Humana members.
  • Utilize your strong nursing background and knowledge to understand and address the unique needs and challenges of our members.
  • Build strong relationships with members by providing personalized support and guidance, serving as a trusted resource for their healthcare needs.
  • Conduct thorough assessments of each member's health, including physical, emotional, and social factors, to develop individualized care plans.
  • Collaborate with healthcare providers to ensure that members receive timely and appropriate care, including coordinating appointments, referrals, and follow-up care.
  • Monitor and track member progress and outcomes, identifying opportunities for improvement and making necessary adjustments to care plans.
  • Educate members on health and wellness topics, empowering them to take an active role in their own healthcare.
  • Stay up-to-date on industry best practices and trends, sharing knowledge with team members and implementing new strategies to improve care navigation processes.
  • Maintain accurate and thorough documentation of all interactions with members and providers.
  • Participate in team meetings and trainings to continuously improve care navigation services and enhance the overall member experience.
  • Uphold Humana's values of compassion, excellence, and integrity in all interactions with members, providers, and team members.

Job Qualifications
  • Active Rn License: The Divisional Rn Care Navigator Must Hold A Current, Active Rn License In The State Of Texas, Specifically For The Houston Area.

  • Minimum Of 3 Years Clinical Experience: Candidates Must Possess At Least 3 Years Of Experience In A Clinical Setting, Preferably In A Remote Or Telehealth Position.

  • Strong Communication And Interpersonal Skills: The Role Of A Divisional Rn Care Navigator Requires Frequent Communication With Patients, Providers, And Other Team Members, Making Strong Communication Skills Essential.

  • Knowledge Of Chronic Disease Management: The Ideal Candidate Should Have A Thorough Understanding Of Chronic Disease Management And Be Able To Utilize This Knowledge To Support Patients In Navigating Their Healthcare Journey.

  • Familiarity With Technology And Remote Tools: As A Remote Position, The Divisional Rn Care Navigator Must Have Experience With Various Technology Platforms And Be Comfortable Using Remote Tools To Provide Patient Care.
