  1. Develop and implement strategic plans for health services and care coordination programs to align with company goals and objectives.
  2. Oversee the day-to-day operations of health services and care coordination teams, including setting goals and objectives, tracking progress, and making necessary adjustments.
  3. Monitor industry trends and regulatory changes to ensure compliance and identify opportunities for program improvements.
  4. Collaborate with internal and external partners to develop and maintain strong relationships and partnerships to enhance our members' care.
  5. Manage and allocate budget and resources effectively to achieve program goals and objectives.
  6. Provide leadership and guidance to team members, fostering a positive and collaborative work environment.
  7. Develop and implement policies and procedures to ensure the delivery of high-quality, cost-effective healthcare services.
  8. Analyze data and performance metrics to identify areas for improvement and develop and implement action plans.
  9. Ensure timely and accurate reporting of program performance to senior leadership and stakeholders.
  10. Stay updated on best practices and advancements in healthcare delivery and apply them to improve the quality of care for our members.
  11. Represent the company at industry conferences and events, promoting our mission and services.
  12. Encourage a culture of continuous learning and professional development within the team.
  13. Collaborate with other departments to support integrated and coordinated care for our members.
  14. Act as a liaison between the company and external healthcare providers, advocating for the needs and interests of our members.
  15. Uphold company values and ethical standards in all interactions and decision-making processes.

Job Qualifications
  • A Minimum Of 5 Years Of Experience In A Leadership Role Within The Healthcare Industry, Preferably In A Managed Care Organization.

  • A Strong Understanding Of Healthcare Regulations And Policies, Including Medicare And Medicaid.

  • Excellent Communication And Interpersonal Skills, With The Ability To Collaborate With Various Stakeholders Such As Providers, Members, And Internal Teams.

  • Proven Experience In Developing And Implementing Care Coordination Programs To Improve Health Outcomes And Reduce Costs.

  • A Bachelor's Or Master's Degree In A Healthcare-Related Field, Such As Nursing, Healthcare Administration, Or Public Health. A Clinical Background Is Preferred.
