  1. Conduct detailed clinical assessments for home health patients to determine their individual care needs.
  2. Ensure that all assessments are conducted accurately and in a timely manner.
  3. Collaborate with other members of the healthcare team to develop personalized care plans for each patient.
  4. Communicate assessment findings and care plans to patients, their families, and other healthcare professionals.
  5. Continuously monitor and evaluate the effectiveness of care plans and make necessary adjustments.
  6. Maintain accurate and up-to-date documentation of all assessments and care plans.
  7. Act as a liaison between patients, families, and healthcare providers to address any concerns or issues.
  8. Stay updated on industry standards, best practices, and regulations related to home health care.
  9. Participate in quality improvement initiatives and contribute to the overall improvement of care for home health patients.
  10. Provide ongoing education and support to patients and their families regarding their health conditions and care plans.
  11. Serve as an advocate for patients, ensuring they receive the best possible care and support.
  12. Demonstrate compassion, empathy, and a patient-centered approach in all interactions with patients and their families.
  13. Collaborate with insurance companies and other third-party payers to ensure proper coverage and reimbursement for services provided.
  14. Adhere to all company policies, procedures, and ethical standards.
  15. Participate in training and development opportunities to enhance clinical skills and knowledge.

Job Qualifications
  • Bachelor's Degree In Nursing Or A Related Healthcare Field.

  • Minimum Of 3-5 Years Of Experience In A Clinical Assessment Role, Preferably In Home Health Or A Related Field.

  • Current And Active Rn License In The State Of Employment.

  • Knowledge And Understanding Of Medicare And Medicaid Regulations And Guidelines Related To Home Health Services.

  • Strong Communication And Interpersonal Skills, With The Ability To Effectively Collaborate With Interdisciplinary Teams And Communicate With Patients And Their Families.
