  1. Provide compassionate and high-quality care to patients in their homes.
  2. Utilize home health per diem experience to effectively assess and meet the needs of patients.
  3. Communicate effectively with patients, their families, and other healthcare team members.
  4. Follow all company policies and procedures, as well as state and federal regulations.
  5. Administer medications and treatments as prescribed by healthcare providers.
  6. Assist patients with daily living activities, including bathing, dressing, and grooming.
  7. Monitor and document patients' vital signs, progress, and any changes in condition.
  8. Collaborate with other healthcare professionals to develop and implement care plans.
  9. Maintain a safe and clean environment for patients and adhere to infection control protocols.
  10. Provide emotional support and companionship to patients and their families.
  11. Stay up-to-date on current healthcare practices and regulations.
  12. Adhere to ethical and professional standards of conduct.
  13. Attend trainings and meetings as required by the company.
  14. Demonstrate a positive and professional attitude towards patients, colleagues, and the healthcare team.
  15. Continuously strive to improve the quality of care provided to patients.

Job Qualifications
  • Certification: The Candidate Must Hold A Valid Certification As A Certified Nursing Assistant (Cna) From An Accredited Institution Or Program.

  • Experience: Prior Experience As A Cna In A Home Health Setting Is Preferred, But Not Required. The Candidate Should Have A Strong Understanding Of Basic Nursing Skills And Procedures.

  • Compassionate And Empathetic: The Candidate Must Possess A Genuine Sense Of Compassion And Empathy For Patients, As They Will Be Providing Direct Care To Individuals In Their Homes.

  • Communication Skills: Excellent Communication Skills Are Essential For This Role, As The Cna Will Be Responsible For Communicating With Both Patients And Their Families, As Well As The Healthcare Team.

  • Flexibility And Availability: The Candidate Must Be Able To Work A Flexible Schedule And Be Available For Per Diem Shifts, As This Job May Require Working On Weekends, Holidays, Or Evenings Depending On The Needs Of The Patients.
