  1. Coordinate and manage the healthcare needs of Humana members at the Weston location.

  4. Conduct thorough assessments of members' health and develop personalized care plans.
  5. Monitor and track members' progress and make necessary adjustments to their care plans.

  8. Educate members and their families about their healthcare options and resources.

  11. Uphold high ethical standards and maintain confidentiality of member information.
  12. Participate in team meetings and contribute to the overall success of the department.

Job Qualifications
