- Provide telephonic support and care coordination for Humana members on a PRN basis, working evenings and weekends.
- Utilize clinical nursing knowledge and skills to assess and evaluate members' health needs.
- Collaborate with members, their families, and healthcare providers to develop and implement personalized care plans.
- Monitor and track members' progress, making necessary adjustments to care plans as needed.
- Educate members on disease management, preventive care, and healthy lifestyle choices.
- Coordinate with other healthcare professionals to ensure seamless and efficient delivery of care.
- Document all interactions and interventions accurately and timely.
- Follow HIPAA guidelines and maintain confidentiality in all member interactions.
- Continuously evaluate and improve care processes to enhance the quality of care provided.
- Stay updated on industry best practices and relevant healthcare policies and regulations.
- Serve as an advocate for members, ensuring they receive the best possible care and support.
- Collaborate with other members of the Humana team to achieve organizational goals and objectives.
- Maintain a compassionate and caring demeanor while interacting with members.
- Communicate effectively and professionally with members, their families, and healthcare providers.

Bachelor's Degree In Nursing: A Care Manager- Telephonic Nurse Must Hold A Bachelor's Degree In Nursing From An Accredited Institution. This Ensures That The Candidate Has A Thorough Understanding Of Healthcare Practices And Patient Care.
Active Rn License: The Candidate Must Possess A Valid And Active Registered Nurse (Rn) License In The State Where They Will Be Working. This Ensures That The Candidate Is Legally Allowed To Practice Nursing And Provide Patient Care.
Prior Experience In Care Management: Humana Seeks Candidates With Prior Experience In Care Management, Preferably In A Telephonic Setting. This Experience Demonstrates The Candidate's Ability To Effectively Assess And Manage Patient Needs Remotely.
Strong Communication Skills: As A Telephonic Nurse, The Candidate Must Have Excellent Communication Skills To Effectively Interact With Patients, Their Families, And Other Healthcare Providers. This Includes Active Listening, Empathy, And Clear And Concise Verbal And Written Communication.
Availability For Evening And Weekend Shifts: The Job Description Specifies That The Position Is For Prn (As Needed) Evenings And Weekends.
