  1. Provide compassionate and personalized care to members over the phone.
  2. Collaborate with members, their families, and healthcare providers to develop and implement individualized care plans.
  3. Monitor and assess the health and well-being of members, identifying any potential issues or gaps in care.
  4. Coordinate and facilitate communication between members and their healthcare team to ensure continuity of care.
  5. Educate members and their families on their health conditions, treatment options, and self-management techniques.
  6. Utilize critical thinking skills to identify and address any barriers to care.
  7. Document all interactions and interventions accurately and in a timely manner.
  8. Maintain strict confidentiality and adhere to all HIPAA regulations.
  9. Adhere to Humana's policies and procedures, as well as industry standards and best practices.
  10. Stay up-to-date on healthcare trends and advancements in order to provide the best possible care to members.
  11. Collaborate with other members of the care team to ensure seamless delivery of care.
  12. Act as an advocate for members, ensuring their needs and preferences are considered in all aspects of care.
  13. Communicate effectively with team members, both verbally and in writing.
  14. Participate in ongoing training and professional development opportunities to enhance nursing skills and knowledge.
  15. Demonstrate a commitment to providing exceptional patient care and promoting a positive experience for members.
  16. Maintain a positive and professional attitude at all times, representing Humana in a positive light.
  17. Adhere to all ethical and legal standards, as well as company policies and procedures.
  18. Continuously evaluate and improve processes and procedures to enhance the overall member experience.
  19. Maintain accurate and up-to-date records and reports for tracking and reporting purposes.
  20. Collaborate with other departments within the company to ensure the best possible outcomes for members.

Job Qualifications
  • Active Nursing License: The Care Manager, Telephonic Nurse Should Have A Current And Active Nursing License In The State In Which They Will Be Working. This Ensures That They Have The Necessary Education And Training To Provide Clinical Support And Guidance To Patients.

  • Experience In Care Management: Candidates Should Have At Least 2-3 Years Of Experience In Care Management, Preferably In A Telephonic Setting. This Experience Should Include Developing Care Plans, Coordinating Care With Providers And Other Healthcare Professionals, And Providing Patient Education And Support.

  • Strong Communication Skills: As A Telephonic Nurse, Excellent Communication Skills Are Essential. The Care Manager Should Be Able To Effectively Communicate With Patients, Healthcare Providers, And Other Team Members To Ensure Quality Care And Coordination.

  • Knowledge Of Medicare And Medicaid: Humana Is A Leading Provider Of Medicare And Medicaid Plans, So It Is Essential That The Care Manager Has A Strong Understanding Of These Programs. This Includes Knowledge Of Coverage, Eligibility Requirements, And Regulations.

  • Computer Skills: As Most Of The Work Will Be Done Remotely, The Care Manager Should Have Proficient Computer Skills, Including Familiarity With Electronic Medical Records And Telehealth Platforms. They Should Also Be Comfortable Navigating Various Software Programs And Tools To Document And Track Patient Care.
