Care Manager, LCSW

4. Monitor and evaluate the effectiveness of care plans and make adjustments as needed.
5. Provide ongoing support and education to members and their families to promote self-management of their healthcare needs.
6. Advocate for members' needs and rights within the healthcare system.
7. Collaborate with community organizations and resources to ensure members have access to necessary support services.
8. Maintain accurate and timely documentation of all interactions and interventions with members.
9. Adhere to all ethical and legal standards in the provision of care.
10. Stay informed about current trends and best practices in social work and healthcare to continuously improve the quality of care provided.
11. Participate in team meetings and case conferences to discuss member progress and develop treatment plans.
12. Communicate effectively and professionally with members, their families, and other healthcare professionals.
13. Handle crisis situations and provide appropriate support and interventions.
