Social Worker - Collaborative Care

We are looking for an experienced Social Worker to join Houston Methodist's Collaborative Care team and help us to provide the highest quality of care to patients and their families. The successful candidate will have a passion for helping others and a commitment to delivering compassionate, professional care.The ideal candidate should have a Master's degree in Social Work and be a Licensed Clinical Social Worker in the state of Texas. Previous experience in a hospital setting is preferred. The ideal candidate should also possess excellent communication and interpersonal skills, as well as a demonstrated ability to work independently and collaboratively as part of a multidisciplinary team. Responsibilities:

1. Provide quality care to patients and their families in accordance with Houston Methodist's Collaborative Care standards.
2. Develop and maintain positive relationships with patients, families, and other members of the healthcare team.
3. Provide individual and family counseling services as needed.
4. Develop and implement patient care plans based on evidence-based practice and current best practices.
5. Ensure all documentation is completed in a timely and accurate manner.
6. Participate in multidisciplinary team meetings and provide input and feedback.
7. Provide crisis intervention services as needed.
8. Maintain up-to-date knowledge of relevant laws and regulations related to social work practice.
9. Attend training and continuing education programs as needed.
10. Participate in research projects and initiatives as needed.
