- Oversee and manage the daily operations of the case management and social services departments at the Willowbrook location.
- Develop and implement strategic plans and goals for the departments in alignment with the overall mission and vision of Houston Methodist.
- Ensure the delivery of exceptional patient care by providing guidance, support, and direction to the case management and social services teams.
- Collaborate with other departments and healthcare professionals to develop and implement individualized care plans for patients.
- Serve as an advocate for patients and their families, ensuring their needs are met and their voices are heard.
- Develop and maintain relationships with community organizations and resources to provide additional support and resources for patients.
- Monitor and maintain department budgets, ensuring efficient use of resources and adherence to financial goals.
- Stay up-to-date on industry trends and best practices in case management and social services, and incorporate them into department operations.
- Train, supervise, and evaluate department staff to ensure high-quality performance and professional development.
- Collaborate with the leadership team to identify and address any issues or barriers that may impact the delivery of patient care.
- Ensure compliance with all regulatory and accreditation standards related to case management and social services.
- Foster a positive and inclusive work environment that promotes teamwork, open communication, and professional growth.
- Represent Houston Methodist at professional conferences, events, and meetings to promote the organization and build relationships with potential partners.
- Continuously monitor and evaluate department processes and procedures to identify areas for improvement and implement necessary changes.
- Uphold the values and ethical standards of Houston Methodist in all job duties and interactions with patients, staff, and the community.
Minimum Of 5 Years Of Experience In Healthcare Case Management And Social Services, Preferably In A Hospital Setting.
Master's Degree In Social Work, Nursing, Or Related Field.
Current Licensure Or Certification In Relevant Field (E.g. Lcsw, Rn, Etc.).
Strong Leadership And Management Skills, With Experience In Overseeing A Team Of Case Managers And Social Workers.
Extensive Knowledge Of Healthcare Regulations And Reimbursement Policies, Including Medicare And Medicaid.
