- Conduct patient assessments to determine their needs and develop a comprehensive care plan.
- Communicate with patients, families, and healthcare professionals to coordinate and facilitate the delivery of quality care.
- Provide guidance and support to patients and families regarding available resources and services.
- Maintain accurate and up-to-date records of patient progress and care plans.
- Collaborate with insurance companies and other third-party payers to obtain authorization for necessary services.
- Monitor and evaluate patient progress and make necessary adjustments to care plans.
- Advocate for patients and ensure their needs and preferences are considered in the care process.
- Participate in multidisciplinary team meetings to discuss patient care and make recommendations for improvement.
- Educate patients and families on their rights and responsibilities in the healthcare system.
- Follow all legal and ethical guidelines in patient care and maintain confidentiality at all times.
- Keep abreast of changes in healthcare laws and regulations to ensure compliance.
- Provide support to patients and families during difficult times and assist in making end-of-life care decisions.
- Participate in continuous education and training opportunities to enhance knowledge and skills.
- Maintain a positive and professional attitude and contribute to a collaborative and supportive work environment.
- Promote a culture of patient-centered care and strive to continuously improve the patient experience.
Strong Communication And Interpersonal Skills.
Bachelor's Degree In Social Work, Nursing, Psychology, Or A Related Field.
At Least 2 Years Of Experience In Case Management Or Healthcare Coordination.
Knowledge Of Healthcare Laws And Regulations, Including Hipaa.
Ability To Multitask And Prioritize Effectively In A Fast-Paced Environment.
