  1. Conduct patient assessments to determine their needs and develop a comprehensive care plan.
  2. Communicate with patients, families, and healthcare professionals to coordinate and facilitate the delivery of quality care.
  3. Provide guidance and support to patients and families regarding available resources and services.
  4. Maintain accurate and up-to-date records of patient progress and care plans.
  5. Collaborate with insurance companies and other third-party payers to obtain authorization for necessary services.
  6. Monitor and evaluate patient progress and make necessary adjustments to care plans.
  7. Advocate for patients and ensure their needs and preferences are considered in the care process.
  8. Participate in multidisciplinary team meetings to discuss patient care and make recommendations for improvement.
  9. Educate patients and families on their rights and responsibilities in the healthcare system.
  10. Follow all legal and ethical guidelines in patient care and maintain confidentiality at all times.
  11. Keep abreast of changes in healthcare laws and regulations to ensure compliance.
  12. Provide support to patients and families during difficult times and assist in making end-of-life care decisions.
  13. Participate in continuous education and training opportunities to enhance knowledge and skills.
  14. Maintain a positive and professional attitude and contribute to a collaborative and supportive work environment.
  15. Promote a culture of patient-centered care and strive to continuously improve the patient experience.

Job Qualifications
  • Strong Communication And Interpersonal Skills.

  • Bachelor's Degree In Social Work, Nursing, Psychology, Or A Related Field.

  • At Least 2 Years Of Experience In Case Management Or Healthcare Coordination.

  • Knowledge Of Healthcare Laws And Regulations, Including Hipaa.
