Job Description

We are looking for an enthusiastic and patient-focused Care Coordinator to join our team at Houston Methodist! This position plays an integral role in providing high quality care to our patients and promoting positive collaborative relationships with their healthcare team.The ideal candidate for this position is a self-motivated professional with excellent communication and organizational skills. They must be able to prioritize tasks and multi-task in a fast-paced environment while maintaining superior customer service.To be successful in this role, the Care Coordinator must have the following qualifications: a Bachelor’s degree in a healthcare related field, a minimum of 1 year of experience in a healthcare setting, a valid nursing license, and experience with EHR systems.

Responsibilities:

  1. Provide high quality care to patients in accordance with established standards and protocols.
  2. Ensure timely and accurate completion of all patient care documentation.
  3. Maintain open and collaborative communication with the patient’s healthcare team.
  4. Monitor patient progress and coordinate care with other healthcare providers.
  5. Assist in developing patient care plans and goals.
  6. Provide patient education and counseling on healthcare topics.
  7. Keep up to date on changes in healthcare regulations and policies.
  8. Assist with the development and implementation of patient care policies and procedures.
  9. Monitor performance of healthcare staff and provide feedback as needed.
  10. Use EHR systems to access and maintain patient records.
