The Nephrology Social Worker will be responsible for working with patients, families, and healthcare team members to provide emotional and psychosocial support and to ensure the patient's medical and psychosocial needs are met. Responsibilities: • Provide emotional and psychosocial support to patients, families, and healthcare team members • Evaluate patient and family needs related to chronic renal disease • Develop and implement individualized care plans for patients • Coordinate patient care with other healthcare team members • Provide education and counseling on topics such as lifestyle changes, coping strategies, and end-of-life care • Maintain accurate patient records and documentation • Follow up with patients to ensure their needs are being met • Participate in regular meetings with the healthcare team to provide updates on patient progress • Participate in professional development activities to stay up-to-date on best practices in Nephrology social work
