Manager - Social Work Services

Responsibilities:

1. Develop and maintain relationships with external stakeholders to ensure positive outcomes for patients and families.
2. Collaborate with a diverse team to ensure a high level of quality care for patients and families.
3. Manage multiple priorities and tasks in a timely and efficient manner.
4. Utilize strong verbal and written communication skills to effectively communicate with patients and families.
5. Provide compassionate, patient-centered care to both patients and families.
6. Monitor patient care plans and make necessary changes to ensure quality care.
7. Monitor and evaluate patient outcomes to ensure success.
8. Develop and implement policies and procedures related to social work services.
9. Develop and implement educational and training programs for social work staff.
10. Provide guidance and support to social work staff in the development of individual patient care plans.
