Facility Master Social Worker

We’re looking for an experienced and compassionate Facility Master Social Worker to join our team at Fresenius Medical Care! As our Facility Master Social Worker, you’ll be responsible for providing psychosocial services to our patients, including counseling and referral services. The ideal candidate for this role will possess both a strong clinical background and excellent interpersonal skills. Responsibilities:

1. Provide psychosocial services, including counseling and referral services, to patients.
2. Develop and maintain effective relationships with patients and their families.
3. Maintain a working knowledge of community resources and utilize them as needed.
4. Collaborate with physicians and other healthcare professionals to ensure effective patient care.
5. Document patient progress and chart notes.
6. Monitor patient outcomes and adjust care plans as necessary.
7. Participate in healthcare team meetings and provide input as needed.
8. Ensure compliance with all relevant healthcare regulations and laws.
9. Remain up-to-date on current trends and best practices in the field of social work.
10. Participate in continuing education and professional development activities.
