- Coordinate and manage patient care plans, ensuring they align with the goals and objectives of DaVita Kidney Care.
- Collaborate with healthcare professionals, including physicians and nurses, to ensure the best possible care for patients.
- Conduct regular check-ins and follow-ups with patients to monitor their progress and address any concerns or issues.

- Adhere to all applicable privacy and confidentiality regulations and guidelines in handling patient information.
- Collaborate with other members of the healthcare team to improve the overall quality of care and patient outcomes.
- Participate in team meetings and contribute ideas and suggestions for improving processes and patient care.
- Maintain a high level of professionalism and integrity in all interactions with patients, colleagues, and other stakeholders.
- Adhere to all policies, procedures, and protocols of DaVita Kidney Care to ensure the highest standards of patient care and safety.
Bachelor's Degree In Social Work, Nursing, Or A Related Field.
Minimum Of 2 Years Of Experience In Case Management, Preferably In A Healthcare Setting.
Knowledge And Understanding Of Chronic Kidney Disease And Its Treatment.
Excellent Communication And Interpersonal Skills To Effectively Coordinate Care With Patients, Families, And Healthcare Team Members.
Strong Organizational Skills And Ability To Prioritize Tasks And Manage A Caseload Of Patients.
