1. Provide emotional support and counseling to patients and their families during their time in the DaVita Kidney Care program.
2. Collaborate with the interdisciplinary team to develop individualized care plans for each patient, taking into consideration their social, emotional, and financial needs.
3. Conduct psychosocial assessments, identifying potential barriers to treatment and developing solutions to address them.
4. Connect patients with community resources and support services, such as financial assistance programs, transportation services, and support groups.
5. Advocate for patients and their families, ensuring their needs are met and their rights are protected.
6. Educate patients and families about their condition, treatment options, and self-care techniques to improve their quality of life.
7. Document all patient interactions and maintain accurate and thorough records.
8. Participate in team meetings and provide input on patient care plans and progress.
9. Collaborate with other social workers and healthcare professionals to continuously improve the quality of care provided to patients.
10. Stay informed about changes in laws, regulations, and policies related to social work and healthcare, and ensure compliance with all regulations.
11. Maintain professional and ethical standards in all interactions with patients, families, and coworkers.
12. Act as a liaison between patients and healthcare providers, advocating for the patients' needs and concerns.
13. Continuously evaluate and assess the effectiveness of social work interventions and make necessary adjustments to ensure the best possible outcomes for patients.
14. Participate in ongoing professional development and training to stay updated on best practices and new developments in the field of social work.
