  1. Provide emotional support and counseling to patients and their families facing the challenges of kidney disease.
  2. Conduct thorough assessments of patients' social, emotional, and financial needs and develop individualized care plans.
  3. Collaborate with interdisciplinary team members to ensure coordinated and comprehensive care for patients.
  4. Connect patients with community resources and support services to enhance their quality of life.
  5. Advocate for patients and their families to ensure their needs and concerns are addressed.
  6. Maintain accurate and timely documentation of patient interactions and progress.
  7. Educate patients and their families about kidney disease, treatment options, and self-management strategies.
  8. Participate in discharge planning and transition of care for patients returning home or to another care setting.
  9. Adhere to all ethical and professional standards in accordance with the NASW Code of Ethics.
  10. Attend and actively participate in team meetings and continuing education opportunities to stay up-to-date on best practices and policies in kidney care.
  11. Foster a positive and supportive work environment by collaborating with team members and promoting a culture of compassion and teamwork.
  12. Comply with all company policies and procedures, as well as federal and state regulations.
  13. Maintain confidentiality of patient information and respect their privacy at all times.
  14. Continuously evaluate and improve social work practices to enhance the overall patient experience.
  15. Act as a liaison between patients, families, and healthcare providers to facilitate effective communication and resolve any issues or concerns.

Job Qualifications
  • Master's Degree In Social Work (Msw) From An Accredited University.

  • State Licensure As A Licensed Clinical Social Worker (Lcsw).

  • Minimum Of 2 Years Of Experience In A Healthcare Or Medical Setting, Preferably In Nephrology Or Dialysis.

  • Strong Knowledge And Understanding Of Social Work Principles, Ethics, And Practices.

  • Excellent Communication And Interpersonal Skills, With The Ability To Work Effectively With Patients, Families, And Interdisciplinary Healthcare Teams.
