1. Provide psychosocial support to patients and their families by conducting individual and family assessments and developing personalized care plans.
2. Collaborate with the multidisciplinary team to address the social, emotional, and financial needs of patients.
3. Offer counseling, crisis intervention, and emotional support to patients and their families.
4. Connect patients and families with community resources and support services to improve their overall well-being.
5. Advocate for patients' rights and assist in resolving any issues or concerns they may have.
6. Educate patients and their families about chronic kidney disease and the importance of self-care and treatment adherence.
7. Monitor and document patients' progress and provide ongoing support and guidance as needed.
8. Participate in discharge planning and assist patients with transitioning to home or other care settings.
9. Collaborate with insurance companies and other healthcare providers to ensure patients have access to necessary resources and services.
10. Stay up-to-date on best practices in social work and incorporate them into daily practice to enhance patient care.
11. Maintain accurate and timely patient records and documentation.
12. Act as a liaison between patients, families, and the healthcare team to facilitate effective communication and coordination of care.
