- Provide psychosocial support to patients and their families by conducting individual and family assessments and developing personalized care plans.
- Collaborate with the multidisciplinary team to address the social, emotional, and financial needs of patients.
- Offer counseling, crisis intervention, and emotional support to patients and their families.
- Connect patients and families with community resources and support services to improve their overall well-being.
- Advocate for patients' rights and assist in resolving any issues or concerns they may have.
- Educate patients and their families about chronic kidney disease and the importance of self-care and treatment adherence.
- Monitor and document patients' progress and provide ongoing support and guidance as needed.
- Participate in discharge planning and assist patients with transitioning to home or other care settings.
- Collaborate with insurance companies and other healthcare providers to ensure patients have access to necessary resources and services.
- Stay up-to-date on best practices in social work and incorporate them into daily practice to enhance patient care.
- Maintain accurate and timely patient records and documentation.
- Act as a liaison between patients, families, and the healthcare team to facilitate effective communication and coordination of care.
- Attend team meetings and contribute to the development of patient care plans.
- Adhere to all ethical and legal standards of practice.
- Participate in professional development and continuous learning opportunities to enhance skills and knowledge.
Master's Degree In Social Work (Msw): A Minimum Of A Master's Degree In Social Work Is Required For This Position At Davita Kidney Care. This Degree Provides The Necessary Knowledge And Skills To Effectively Work With Patients And Their Families In A Healthcare Setting.
State And National Licensure: A Valid State License As A Licensed Clinical Social Worker (Lcsw) Is Required For This Job. Additionally, National Certification Through The National Association Of Social Workers (Nasw) Is Preferred.
Experience In Healthcare Setting: Previous Experience Working In A Healthcare Setting, Preferably With Patients With Chronic Conditions, Is Highly Desired For This Position. This Experience Allows For A Better Understanding Of The Unique Needs And Challenges Faced By Patients In A Medical Setting.
Knowledge Of Kidney Disease And Treatment: A Strong Understanding Of Kidney Disease, Dialysis Treatment, And Other Related Medical Conditions Is Essential For This Role. This Knowledge Allows For Effective Communication And Support For Patients And Their Families.
Strong Communication And Interpersonal Skills: The Ability To Communicate Effectively And Build Relationships With Patients, Their Families, And Interdisciplinary Healthcare Teams Is Crucial For Success In This Job. Social Workers Must Be Able To Handle Sensitive Information And Navigate Difficult Conversations With Empathy And Professionalism.
