  1. Conduct comprehensive assessments of patients' social, emotional, and psychological needs.
  2. Collaborate with the interdisciplinary team to develop and implement individualized treatment plans for patients.
  3. Provide counseling and support to patients and their families to help them cope with the challenges of chronic illness.
  4. Advocate for patients' rights and facilitate access to community resources and services.
  5. Maintain accurate and timely documentation of all patient interactions and interventions.
  6. Educate patients and their families about the disease process, treatment options, and self-care strategies.
  7. Monitor and evaluate the effectiveness of interventions and make necessary adjustments to treatment plans.
  8. Participate in interdisciplinary team meetings and provide input to improve patient care.
  9. Stay current with best practices and trends in social work and incorporate them into practice.
  10. Uphold ethical standards and maintain confidentiality of patient information.
  11. Collaborate with community organizations and agencies to provide support and resources to patients.
  12. Serve as a liaison between patients, families, and healthcare providers to ensure effective communication and coordination of care.
  13. Provide crisis intervention and support during difficult situations.
  14. Advocate for social justice and promote policies that benefit the well-being of patients.
  15. Participate in training and development opportunities to enhance clinical skills and knowledge.

Job Qualifications
  • State Licensure As A Licensed Clinical Social Worker (Lcsw).

  • Master's Degree In Social Work (Msw) From An Accredited Program.

  • Minimum Of 2 Years Experience In A Healthcare Setting, Preferably In Nephrology Or Dialysis.

  • Knowledge And Understanding Of Medicare And Medicaid Regulations And Policies.

  • Strong Communication And Interpersonal Skills, Ability To Work Effectively With Diverse Populations And Collaborate With Interdisciplinary Team Members.
