As a Social Worker with a Master of Social Work (MSW) degree, you will play a crucial role in our patient-centered approach to care.   1. Provide comprehensive social work services to patients and families affected by chronic kidney disease.
  2. Conduct initial assessments and develop individualized care plans based on patient needs and goals.
  3. Collaborate with interdisciplinary team members to coordinate and facilitate patient care, including medical, emotional, and financial needs.
  4. Offer emotional support and counseling to patients and their families, helping them navigate the challenges of chronic kidney disease.
  5. Assist patients in accessing community resources and support services, such as transportation, housing, and financial assistance.
  6. Advocate for patients' rights and ensure they receive the highest quality of care.
  7. Educate patients and families on managing their condition, including adherence to treatment plans, diet and lifestyle changes, and coping strategies.
  8. Monitor and evaluate patient progress and make necessary adjustments to care plans.
  9. Maintain accurate and timely documentation of all patient interactions and interventions.
  10. Participate in team meetings, trainings, and other professional development opportunities to enhance knowledge and skills.
  11. Adhere to ethical and professional standards of social work practice.
  12. Uphold the company's values of teamwork, integrity, and continuous growth in all interactions and responsibilities.

Job Qualifications
  • Master's Degree In Social Work (Msw) From An Accredited Institution

  • Active State Licensure As A Licensed Clinical Social Worker (Lcsw)

  • Minimum Of 2 Years Of Experience In A Healthcare Setting, Preferably With A Focus On Chronic Illness And/Or Kidney Disease

  • Knowledge And Understanding Of Federal And State Regulations Related To Social Work Practice

  • Strong Communication, Problem-Solving, And Interpersonal Skills To Effectively Work With Patients, Families, And Interdisciplinary Healthcare Team Members
