Job Description

  1. Conduct assessments: Conduct initial and ongoing assessments of patients' psychosocial needs, including emotional, financial, and practical support.
  2. Develop treatment plans: Collaborate with patients, families, and members of the healthcare team to develop comprehensive treatment plans that address the patients' social, emotional, and financial needs.
  3. Provide counseling: Offer individual and group counseling to patients and their families to address psychosocial issues and promote coping skills.
  4. Assist with resource coordination: Connect patients with community resources and support programs to address their social and financial needs.
  5. Advocate for patients: Serve as an advocate for patients' rights and needs within the healthcare system and assist with navigating healthcare resources and processes.
  6. Collaborate with interdisciplinary team: Work closely with physicians, nurses, and other healthcare professionals to ensure coordinated and comprehensive patient care.
  7. Monitor patient progress: Continuously monitor and reassess patients' psychosocial needs and adjust treatment plans as necessary.
  8. Provide education: Educate patients and their families about chronic kidney disease, treatment options, and self-management techniques.
  9. Maintain accurate records: Record all patient interactions, assessments, and interventions accurately and in a timely manner.
  10. Stay informed: Stay up-to-date on current trends and developments in the field of social work and chronic kidney disease to continuously improve patient care.
  11. Adhere to ethical standards: Follow ethical standards and guidelines set by the National Association of Social Workers (NASW) and the company.
  12. Participate in team meetings: Attend and actively participate in team meetings to discuss patient progress, treatment plans, and any other relevant topics.
  13. Collaborate with outside agencies: Coordinate with outside agencies and organizations to provide additional support and resources for patients.
  14. Maintain confidentiality: Ensure patient privacy and confidentiality at all times.

Job Qualifications
  • Master's Degree In Social Work (Msw) From An Accredited University: This Is The Primary Qualification For A Social Worker At Davita Kidney Care. A Master's Degree In Social Work Is Necessary To Provide Comprehensive And Specialized Care To Patients With Chronic Kidney Disease.
