- Provide direct social work services to patients and their families, including psychosocial assessments, counseling, and referrals to community resources.
- Collaborate with the interdisciplinary team to develop and implement individualized care plans for patients.
- Advocate for patients and their families to ensure their needs and goals are being met.
- Conduct home visits and participate in multidisciplinary team meetings to assess and address the social and emotional needs of patients.
- Facilitate support groups and educational sessions for patients and their families.
- Serve as a liaison between patients, families, and healthcare providers to ensure effective communication and coordination of care.
- Conduct ongoing evaluations of patients' progress and make adjustments to care plans as needed.
- Maintain accurate and timely documentation of all social work services provided.
- Stay up-to-date on industry trends and best practices in kidney disease treatment and social work.
- Participate in continuous quality improvement initiatives to enhance patient care and outcomes.
- Adhere to all ethical and professional standards set forth by the National Association of Social Workers.
- Collaborate with community organizations to develop and maintain resources for patients and their families.
- Provide emotional support and counseling to patients and families during difficult times.
- Educate patients and families on coping mechanisms and strategies to manage the impacts of kidney disease.
- Maintain confidentiality and respect patients' rights and privacy.
Master's Degree In Social Work (Msw): The Candidate Should Possess A Master's Degree In Social Work From An Accredited University. This Degree Provides The Necessary Knowledge And Skills To Work As A Social Worker In A Healthcare Setting.
State Licensure: The Candidate Should Hold A Valid License To Practice As A Social Worker In The State Where The Job Is Located. This Ensures That The Candidate Has Met The State's Requirements And Is Qualified To Work In The Field Of Social Work.
