  1. Provide compassionate and comprehensive care to patients with kidney disease.
  2. Utilize your Master of Social Work degree to support and advocate for patients.
  3. Develop and implement individualized treatment plans for patients.
  4. Collaborate with interdisciplinary team members to ensure holistic care for patients.
  5. Conduct psychosocial assessments and provide counseling and support to patients and their families.
  6. Connect patients with community resources and support services.
  7. Maintain accurate and up-to-date documentation of patient interactions and progress.
  8. Serve as a liaison between patients, families, and healthcare providers.
  9. Participate in team meetings and contribute ideas for improving patient care.
  10. Stay current on the latest research and developments in the field of kidney disease and social work.
  11. Comply with all legal and ethical standards in the provision of care.
  12. Demonstrate empathy, understanding, and a non-judgmental attitude towards patients.
  13. Maintain patient confidentiality and privacy at all times.
  14. Handle difficult and sensitive situations with professionalism and sensitivity.
  15. Educate patients and their families about the impact of kidney disease and how to manage it effectively.
  16. Advocate for patients' rights and help them navigate the healthcare system.
  17. Continuously evaluate and improve the effectiveness of social work interventions.
  18. Adhere to all company policies, procedures, and guidelines.
  19. Foster a positive and supportive work environment.
  20. Contribute to a culture of continuous learning and growth within the team.

Job Qualifications
  • Master's Degree In Social Work (Msw) From An Accredited University.

  • Minimum Of 2 Years Experience In A Clinical Social Work Setting, Preferably In Healthcare Or Dialysis.

  • Knowledge And Understanding Of Social Work Principles, Theories, And Practices.

  • Ability To Assess And Address The Psychosocial Needs Of Patients With Chronic Kidney Disease And Their Families.

  • Strong Communication And Collaboration Skills To Work Effectively With A Multidisciplinary Team And Community Resources.
