  1. Provide emotional support and advocacy to patients with chronic kidney disease.
  2. Collaborate with physicians, nurses, and other healthcare professionals to develop holistic care plans for patients.
  3. Utilize social work principles, including crisis intervention, case management, and community resources, to support patients and their families.
  4. Conduct psychosocial assessments and develop treatment plans based on patient needs.
  5. Offer counseling and education to patients and their families on coping strategies, disease management, and available resources.
  6. Monitor and track patient progress and adjust treatment plans accordingly.
  7. Participate in multidisciplinary team meetings to discuss patient care and provide input from a social work perspective.
  8. Maintain accurate and confidential patient records and documentation.
  9. Collaborate with community organizations and resources to ensure patients have access to necessary support services.
  10. Stay current on social work best practices and regulations to provide the highest level of care to patients.
  11. Act as a liaison between patients and healthcare providers to facilitate communication and ensure patient needs are met.
  12. Participate in quality improvement initiatives to enhance patient care and outcomes.
  13. Adhere to all ethical and professional standards of social work practice.
  14. Continuously advocate for patients and their families to improve their overall well-being and quality of life.
  15. Contribute to a positive and collaborative work environment within the healthcare team.

Job Qualifications
  • Master's Degree In Social Work (Msw) From An Accredited University.

  • Current State License As A Social Worker.

  • Minimum Of 2 Years Experience In A Healthcare Or Dialysis Setting.

  • Knowledge And Understanding Of Renal Disease And Its Impact On Patients And Families.

  • Strong Communication And Interpersonal Skills, With The Ability To Work Effectively In A Multidisciplinary Team.
