We are currently seeking a compassionate and dedicated Social Worker (MSW) to join our team. As a Social Worker at DaVita Kidney Care, you will have the opportunity to make a positive impact in the lives of our patients and their families.   1. Provide high-quality care and support to patients living with kidney disease.
  2. Conduct assessments to identify patients' social, emotional, and financial needs.
  3. Develop and implement individualized care plans to address patients' needs and goals.
  4. Collaborate with interdisciplinary team members to coordinate comprehensive patient care.
  5. Offer counseling and support to patients and their families to promote emotional well-being.
  6. Connect patients with community resources and services to enhance their quality of life.
  7. Advocate for patients' rights and resources to ensure optimal care and outcomes.
  8. Document patient progress and interventions in accordance with company policies and procedures.
  9. Participate in team meetings and provide input on patient care plans and strategies.
  10. Stay up-to-date on social work principles and practices through continuing education and training.
  11. Maintain confidentiality and adhere to ethical standards in all interactions with patients and their families.
  12. Serve as a liaison between patients, their families, and healthcare providers to facilitate communication and collaboration.
  13. Support the organization's mission and values by providing compassionate and patient-centered care.
  14. Act as a resource and mentor to other team members, including social work interns and new hires.
  15. Continuously assess and evaluate the effectiveness of interventions and make adjustments as needed.

Job Qualifications
  • Master's Degree In Social Work (Msw) From An Accredited University.

  • State Licensure As A Licensed Clinical Social Worker (Lcsw).

  • Minimum Of 2 Years Experience Working As A Social Worker In A Healthcare Setting, Preferably In A Dialysis Or Nephrology Setting.

  • Knowledge And Understanding Of The Unique Needs And Challenges Faced By Patients With Chronic Kidney Disease.

  • Strong Communication And Interpersonal Skills, With The Ability To Work Collaboratively With A Multidisciplinary Team To Provide Comprehensive Care To Patients.
