Job Description

Welcome to DaVita Kidney Care, one of the largest providers of kidney care services in the United States. We are currently seeking a highly motivated and experienced Senior Director to lead our Chronic Kidney Disease (CKD) Population Health department. As the Senior Director, you will play a critical role in improving the health outcomes of our CKD patients through the development and implementation of innovative population health strategies.   1. Develop and implement innovative population health strategies to improve the health outcomes of our CKD patients.
  2. Lead and manage the Chronic Kidney Disease Population Health department, including setting department goals, establishing metrics for success, and overseeing day-to-day operations.
  3. Collaborate with cross-functional teams to identify and address gaps in current population health programs and develop new initiatives to address these gaps.
  4. Provide strategic direction and leadership to the department, driving a culture of innovation and continuous improvement.
  5. Stay up-to-date on industry trends, advancements in CKD treatment, and population health best practices to ensure our programs are at the forefront of the field.
  6. Develop and maintain relationships with key stakeholders, including healthcare providers, community organizations, and patient advocacy groups.
  7. Work closely with data analysts to identify areas for improvement and track the impact of population health programs on patient outcomes.
  8. Manage and allocate department resources effectively to ensure the successful execution of population health initiatives.
  9. Collaborate with the Human Resources department to recruit, train, and develop a high-performing team of population health professionals.
  10. Represent the organization at conferences, events, and meetings to promote our population health programs and initiatives.
  11. Ensure compliance with all regulations and standards related to population health and CKD care.
  12. Foster a positive and inclusive work environment that promotes teamwork, creativity, and collaboration.
  13. Communicate regularly with senior leadership and provide updates on the progress of population health programs.
  14. Act as a mentor and coach to team members, providing guidance and support to help them grow and develop in their roles.
  15. Continuously evaluate and improve population health programs to ensure they are meeting the needs of our CKD patients and aligning with the organization's overall goals and objectives.

Job Qualifications
  • Bachelor's Or Master's Degree In Healthcare Administration, Public Health, Or Related Field.

  • Minimum Of 8-10 Years Of Experience In Population Health Management, Specifically In Chronic Kidney Disease.

  • Strong Understanding Of Healthcare Regulations And Policies Related To Chronic Kidney Disease Management.

  • Experience In Developing And Implementing Population Health Programs And Initiatives To Improve Outcomes For Patients With Chronic Kidney Disease.

  • Excellent Leadership And Communication Skills With The Ability To Collaborate Effectively With Cross-Functional Teams And External Stakeholders.
