
Patient Educator, TCP/Modality Programs

If you are looking for a challenging and rewarding position that will allow you to make a difference in the lives of those we serve, then this could be the perfect role for you!Required Qualifications: • Bachelor’s degree in nursing, social work, or related field• Minimum of two years of patient education experience • Certified Dialysis Educator (CDE) preferred• Knowledge of chronic kidney disease (CKD), end stage renal disease (ESRD), and dialysis treatments • Ability to work independently and as part of a multidisciplinary team • Excellent organizational and communication skills • Bilingual (Spanish) preferred
Responsibilities:
• Provide comprehensive patient education to patients and their families about chronic kidney disease (CKD), end stage renal disease (ESRD), and dialysis treatments • Develop and implement patient education plans to ensure patients receive the best possible care and support • Collaborate with the multidisciplinary team to ensure quality patient care • Maintain a comprehensive understanding of dialysis treatments and patient education requirements • Monitor and evaluate patient education programs to ensure compliance with regulations • Maintain accurate patient records and data to ensure all patient education is properly documented • Provide resources and support to patients and their families to ensure their needs are addressed • Participate in continuing education activities and professional development opportunities to stay up to date on best practices • Assist in the development of new patient education materials to ensure high quality patient care • Act as a liaison between patients, families, and medical staff to ensure a seamless healthcare experience
Bachelor's Degree In Nursing Or Related Field
Current Registered Nurse Or Related Licensure
Minimum Of Three Years Of Experience In A Healthcare Setting
Strong Communication And Teaching Skills
Ability To Assess Patient Learning Needs
Competency In Microsoft Office Applications
Experience With Database Systems And Utilization Of Patient Education Software
