- Provide compassionate and exceptional care to patients with kidney disease.
- Assist in the development and implementation of individualized care plans for patients.
- Monitor and assess patient's condition, progress, and response to treatments.
- Administer medications and treatments as prescribed by physicians.
- Collaborate with interdisciplinary team to ensure coordinated and comprehensive care for patients.
- Educate patients and their families about kidney disease, treatments, and self-care techniques.
- Maintain accurate and thorough documentation of patient care and progress.
- Adhere to all nursing standards, policies, and procedures.
- Participate in ongoing training and education to enhance knowledge and skills.
- Provide emotional support and comfort to patients and their families.
- Communicate effectively with patients, families, and healthcare team members.
- Maintain a clean and safe patient environment.
- Adhere to infection control and safety protocols.
- Report any changes in patient's condition to the appropriate healthcare provider.
- Act as a patient advocate and ensure patient's rights are respected and upheld.
- Maintain confidentiality of patient information.
- Attend staff meetings and participate in quality improvement initiatives.
- Maintain current LVN license and fulfill continuing education requirements.
- Perform other duties and responsibilities as assigned by the supervisor.
Completion Of An Accredited Licensed Vocational Nurse (Lvn) Program
Current And Active Lvn License In The State Of Employment
Minimum Of 1 Year Of Experience In A Clinical Healthcare Setting, Preferably In Nephrology Or Dialysis
Strong Communication And Interpersonal Skills To Effectively Interact With Patients, Families, And Healthcare Team Members
Knowledge Of And Adherence To State And Federal Regulations And Guidelines Related To Nursing Practice And Patient Care
