- Provide personalized nutrition counseling to patients with kidney disease, taking into consideration their individual medical history, dietary needs, and treatment plan.
- Develop and implement individualized nutrition plans for patients, taking into account their preferences and any dietary restrictions.
- Educate patients and their families on the importance of proper nutrition in managing kidney disease and maintaining overall health.
- Work closely with other members of the healthcare team to coordinate and integrate nutrition care into the patient's overall treatment plan.
- Monitor and track patient progress, making necessary adjustments to nutrition plans as needed.
- Stay up-to-date on the latest research and guidelines related to nutrition and kidney disease, incorporating this knowledge into patient care.
- Collaborate with community resources and organizations to provide additional support and resources for patients and their families.
- Maintain accurate and timely documentation of patient interactions and progress.
- Participate in team meetings and communicate effectively with other healthcare professionals.
- Adhere to all company policies and procedures, as well as state and federal regulations related to nutrition and patient care.
Registered Dietitian (Rd) Credential From The Commission On Dietetic Registration (Cdr).
Bachelor's Degree In Nutrition, Dietetics, Or A Related Field From An Accredited University.
Minimum Of 1-2 Years Of Clinical Experience In Nutrition And Dietetics, Preferably In A Renal Or Dialysis Setting.
Knowledge And Understanding Of Renal Nutrition And The National Kidney Foundation's Kidney Disease Outcomes Quality Initiative (Kdoqi) Guidelines.
Strong Communication And Counseling Skills, With The Ability To Educate And Motivate Patients To Make Dietary Changes For Better Health Outcomes.
