We are currently seeking a highly motivated and experienced individual to join our team as a Clinical Coordinator. As a Clinical Coordinator, you will play a crucial role in overseeing the coordination and management of patient care, while also serving as a mentor and leader to our team of dedicated healthcare professionals. To excel in this role, you must possess strong communication, problem-solving, and leadership skills. Additionally, a minimum of 2 years of clinical experience in a healthcare setting and a Bachelor's degree in Nursing or related field are required.   1. Oversee the coordination and management of patient care within DaVita Kidney Care facilities.
  2. Serve as a mentor and leader to a team of healthcare professionals, providing guidance and support in delivering the highest quality of care.
  3. Communicate effectively with patients, their families, and other healthcare providers to ensure a comprehensive and coordinated approach to treatment.
  4. Utilize problem-solving skills to address any issues or challenges that may arise during the course of patient care.
  5. Monitor and evaluate the effectiveness of patient care plans, making adjustments as needed to ensure optimal outcomes.
  6. Ensure compliance with all relevant healthcare regulations and standards.
  7. Collaborate with other healthcare professionals to develop and implement new policies and procedures to improve patient outcomes.
  8. Stay up-to-date on the latest advancements and best practices in kidney care to continuously improve the quality of care provided.
  9. Conduct ongoing training and development for team members to enhance their clinical skills and knowledge.
  10. Maintain accurate and up-to-date patient records and documentation.
  11. Work closely with the administrative team to manage scheduling, budgeting, and resource allocation.
  12. Provide support and guidance to team members in resolving conflicts and managing challenging situations.
  13. Serve as a liaison between patients, their families, and other healthcare providers to ensure a seamless and positive experience.
  14. Demonstrate strong leadership skills by setting a positive example for team members and promoting a collaborative and supportive work environment.
  15. Uphold the mission and values of DaVita Kidney Care in all aspects of the role.

Job Qualifications
  • Bachelor's Degree In Nursing Or Related Field

  • Minimum Of 3-5 Years Of Experience In A Clinical Setting, Preferably In Nephrology Or Dialysis

  • Knowledge And Understanding Of Medicare And Medicaid Regulations Related To Dialysis Services

  • Strong Leadership And Communication Skills To Effectively Manage And Coordinate A Team Of Healthcare Professionals

  • Current Rn License And Bls Certification
