Job Description

At DaVita Kidney Care, we are looking for a Care Manager who will bring a passion for delivering exceptional care and service to our kidney care patients. Our ideal candidate will be an experienced professional with an eye for detail and an unwavering commitment to providing safe and effective patient care.The successful Care Manager will be responsible for providing direct and supportive care to our patients, coordinating care between various healthcare team members, and ensuring that all aspects of patient care follow DaVita's best practices.The ideal candidate for this position will have an RN or MSW certification with at least two years of experience in a clinical setting. They must also possess excellent communication, problem-solving, and organizational skills. Additionally, they should have an in-depth knowledge of government and healthcare regulations, as well as a commitment to continuous learning and improvement. Lastly, they should have a positive attitude and a compassionate approach when caring for our patients.

Responsibilities:

  1. Provide direct and supportive care to kidney care patients.
  2. Coordinate care between various healthcare team members.
  3. Ensure that all aspects of patient care follow DaVita's best practices.
  4. Maintain a valid RN or MSW certification.
  5. Possess excellent communication, problem-solving, and organizational skills.
