Job Description

We are looking for a dedicated and compassionate individual to join our team as a POST Case Manager at the City of Pittsburgh. In this role, you will have the opportunity to make a positive impact on the lives of individuals in our community who are experiencing homelessness.   1. Provide case management services to individuals experiencing homelessness in the City of Pittsburgh.
  2. Conduct thorough assessments of clients' needs and develop personalized action plans to help them achieve stability and self-sufficiency.
  3. Collaborate with community resources and other agencies to connect clients with housing, employment, healthcare, and other necessary services.
  4. Maintain accurate and up-to-date client records and documentation.
  5. Monitor and track clients' progress towards their goals, and make adjustments to their action plans as needed.
  6. Advocate for clients' needs and rights, and provide support and guidance to help them overcome barriers.
  7. Build and maintain positive relationships with clients, treating them with compassion, respect, and empathy.
  8. Participate in team meetings, training, and professional development opportunities to enhance case management skills and stay updated on best practices.
  9. Maintain confidentiality and abide by all ethical and legal standards in working with clients.
  10. Contribute to the overall mission and goals of the organization by actively participating in program development and improvement initiatives.
  11. Collaborate with other case managers and staff to ensure efficient and effective service delivery.
  12. Communicate regularly with supervisors to report on clients' progress and any challenges or concerns.
  13. Participate in program evaluations and provide feedback to improve services and outcomes.
  14. Stay informed about local resources and programs for individuals experiencing homelessness, and make appropriate referrals.
  15. Serve as a positive and professional representative of the organization in the community.

Job Qualifications
