
POST Case Manager

- Provide case management services to individuals experiencing homelessness in the City of Pittsburgh.
- Conduct thorough assessments of clients' needs and develop personalized action plans to help them achieve stability and self-sufficiency.
- Collaborate with community resources and other agencies to connect clients with housing, employment, healthcare, and other necessary services.
- Maintain accurate and up-to-date client records and documentation.
- Monitor and track clients' progress towards their goals, and make adjustments to their action plans as needed.
- Advocate for clients' needs and rights, and provide support and guidance to help them overcome barriers.
- Build and maintain positive relationships with clients, treating them with compassion, respect, and empathy.
- Participate in team meetings, training, and professional development opportunities to enhance case management skills and stay updated on best practices.
- Maintain confidentiality and abide by all ethical and legal standards in working with clients.
- Contribute to the overall mission and goals of the organization by actively participating in program development and improvement initiatives.
- Collaborate with other case managers and staff to ensure efficient and effective service delivery.
- Communicate regularly with supervisors to report on clients' progress and any challenges or concerns.
- Participate in program evaluations and provide feedback to improve services and outcomes.
- Stay informed about local resources and programs for individuals experiencing homelessness, and make appropriate referrals.
- Serve as a positive and professional representative of the organization in the community.
Excellent Communication And Interpersonal Skills.
Ability To Work Independently And As Part Of A Team To Develop And Implement Effective Case Plans.
Bachelor's Degree In Social Work Or A Related Field.
Minimum Of 2 Years Of Experience In Case Management Or Social Services.
Knowledge Of Local Resources And Services Available To Support Clients.
