
POST Case Manager

- Provide compassionate and empathetic support to individuals who have been impacted by violence and trauma.
- Work collaboratively with a diverse range of individuals and organizations to develop and implement resources and support systems for those affected by violence and trauma.
- Conduct thorough assessments of clients' needs and develop personalized treatment plans.
- Maintain accurate and detailed case records and documentation.
- Serve as a liaison between clients and community resources, such as mental health services, legal assistance, and financial resources.
- Continuously monitor and evaluate clients' progress and make necessary adjustments to treatment plans.
- Provide crisis intervention and support to clients as needed.
- Advocate for clients' rights and needs within the community.
- Collaborate with other members of the POST team to develop and implement strategies for preventing violence and promoting community safety.
- Stay up-to-date on best practices and resources for supporting individuals impacted by violence and trauma.
- Participate in regular team meetings and trainings to enhance knowledge and skills.
- Adhere to all confidentiality and ethical standards in working with clients and their information.
- Maintain professional boundaries and handle difficult situations with sensitivity and professionalism.
- Meet all deadlines and ensure timely completion of tasks.
- Serve as a positive representative of the City of Pittsburgh and the POST program in all interactions with clients, colleagues, and community members.
Excellent Communication And Interpersonal Skills.
Minimum Of 2 Years Experience In Case Management Or Social Services.
Bachelor's Degree In Social Work, Psychology, Or A Related Field.
Knowledge Of Community Resources And Services Available For Individuals In Need.
Ability To Work Independently And As Part Of A Team To Develop And Implement Effective Case Plans.
