
Case Manager, Law Enforcement Assisted Diversion (LEAD)

- Develop and maintain relationships with law enforcement agencies and social service organizations to effectively coordinate services for program participants
- Conduct thorough assessments of program participants to identify individual needs and develop personalized care plans
- Provide crisis intervention and de-escalation techniques to support individuals in difficult situations
- Collaborate with community resources to ensure program participants have access to necessary services such as housing, employment, and healthcare
- Monitor and document participant progress and compliance with program requirements
- Advocate for the rights and needs of program participants within the criminal justice system
- Stay up-to-date on relevant laws, policies, and procedures related to the criminal justice system and social services to inform program operations
- Facilitate group meetings and workshops to provide education and support to program participants
- Maintain accurate and confidential records of all client interactions and progress
- Attend regular meetings and training sessions to enhance knowledge and skills related to case management and social work practices.
Bachelor's Degree In Social Work, Criminal Justice, Or A Related Field.
Prior Experience Working With Individuals Involved In The Criminal Justice System And/Or With Substance Abuse And Mental Health Issues.
Knowledge And Understanding Of Trauma-Informed Care And Harm Reduction Principles.
Strong Communication And Interpersonal Skills To Effectively Work With A Diverse Population And Collaborate With Law Enforcement And Community Partners.
Ability To Maintain Accurate And Detailed Case Records, Track Client Progress, And Report Outcomes To Program Supervisors And Funders.
