
Case Manager, Law Enforcement Assisted Diversion (LEAD)

- Develop and maintain relationships with law enforcement agencies, social service providers, and community organizations to support the goals of the LEAD program.
- Conduct assessments and develop individualized treatment plans for clients experiencing substance abuse and mental health issues.
- Collaborate with clients to identify and connect them with appropriate resources and services, such as counseling, housing, and job training.
- Advocate for clients' needs and rights within the criminal justice system and other institutions.
- Monitor clients' progress and provide ongoing support and guidance to assist them in achieving their goals.
- Maintain accurate and confidential client records, documentation, and reports.
- Participate in regular meetings and trainings to stay informed about best practices and updates in the field of substance abuse and mental health.
- Serve as a liaison between clients and their families, as well as other community support systems.
- Work closely with law enforcement to facilitate diversion of individuals with substance abuse and mental health issues from the criminal justice system into treatment and support services.
- Continuously evaluate and assess the effectiveness of the program and make recommendations for improvements.
Bachelor's Degree In Social Work, Criminal Justice, Or Related Field.
Minimum Of 2 Years Of Experience Working In A Case Management Role, Preferably In A Criminal Justice Setting.
Knowledge Of And Experience With Harm Reduction Principles And Evidence-Based Practices In Criminal Justice Interventions.
Strong Communication And Interpersonal Skills, With The Ability To Engage And Build Rapport With Diverse Populations.
Understanding Of The Lead Model And Its Implementation In A Law Enforcement Setting, As Well As Knowledge Of Local Resources And Services For Individuals Experiencing Homelessness Or Substance Use Disorders.
