
Clinical Case Manager Supervisor (Grant-Funded)

- Oversee the grant-funded program and ensure compliance with all program requirements and guidelines.
- Provide leadership and mentorship to a team of case managers, including setting performance expectations and conducting regular evaluations.
- Utilize clinical expertise to guide and support case managers in providing comprehensive services to individuals in need.
- Collaborate with community partners and stakeholders to identify and address gaps in services and resources for vulnerable populations.
- Develop and implement program policies and procedures to ensure efficient and effective service delivery.
- Monitor and track program outcomes and make recommendations for improvement as needed.
- Conduct regular trainings and workshops for case managers to enhance their skills and knowledge.
- Act as a liaison between the program and other city departments, as well as external agencies and organizations.
- Provide direct case management services to clients as needed.
- Maintain accurate and up-to-date documentation of all client interactions and services provided.
- Stay informed of current trends and best practices in case management and incorporate them into program operations.
- Attend meetings, conferences, and trainings related to the field of case management.
- Adhere to all ethical and professional standards in accordance with the National Association of Social Workers (NASW) Code of Ethics.
- Uphold the values and mission of the City of Austin and the program.
- Other duties as assigned by the program director.
Bachelor's Degree In Social Work, Psychology, Or A Related Field.
Minimum Of 5 Years Of Experience In Case Management, Preferably In A Clinical Setting.
Previous Supervisory Experience, Including Managing And Evaluating A Team Of Case Managers.
Knowledge Of Grant-Funded Programs And Experience With Grant Reporting And Compliance.
Strong Communication And Interpersonal Skills, With The Ability To Collaborate With Multiple Stakeholders And Community Partners.
