  1. Conduct comprehensive assessments of clients’ needs, including mental health and substance abuse issues, to develop individualized treatment plans.
  2. Collaborate with clients to identify their goals and develop strategies to achieve them.
  3. Provide ongoing support and guidance to clients to help them overcome barriers and achieve their objectives.
  4. Utilize evidence-based practices and techniques to address clients’ mental health and substance abuse concerns.
  5. Monitor and evaluate clients’ progress towards treatment goals, making adjustments to treatment plans as needed.
  6. Maintain accurate and up-to-date client records, documenting all interactions and progress.
  7. Coordinate care with other service providers, such as healthcare professionals and social workers, to ensure holistic and comprehensive support for clients.
  8. Advocate for clients and connect them with community resources and support systems.
  9. Facilitate group therapy sessions and educational workshops to promote clients’ understanding and management of their mental health and substance abuse disorders.
  10. Participate in team meetings and case conferences to discuss client progress and collaborate on treatment approaches.
  11. Adhere to ethical and legal standards in all aspects of work, including maintaining confidentiality and respecting clients’ rights.
  12. Continuously seek professional development opportunities to enhance skills and knowledge in the field of mental health and substance abuse treatment.
  13. Represent the City of Austin and promote its mission and values in all interactions with clients, colleagues, and community partners.
  14. Provide crisis intervention and support as needed, including after-hours availability for emergencies.
  15. Serve as a positive role model and advocate for mental health and substance abuse awareness and destigmatization.

Job Qualifications
  • Bachelor's Or Master's Degree In Social Work, Psychology, Nursing, Or A Related Field.

  • Minimum Of 2 Years Of Experience In Case Management, Preferably In A Clinical Setting.

  • Knowledge Of Local Resources And Support Services For Individuals With Mental Health Or Substance Abuse Issues.

  • Excellent Communication And Interpersonal Skills, With The Ability To Work With Diverse Populations.

  • Strong Organizational And Time-Management Skills, With The Ability To Prioritize And Manage Multiple Cases Effectively.
