1. Conduct thorough assessments of individuals and families to determine their specific needs and develop individualized treatment plans.
2. Provide advocacy and support for individuals and families in navigating the healthcare system and accessing necessary resources.
3. Collaborate with interdisciplinary team members, including physicians, nurses, and therapists, to ensure comprehensive care for patients.
4. Utilize evidence-based interventions and techniques to address social, emotional, and behavioral issues.
5. Maintain accurate and up-to-date records of client progress and treatment plans.
6. Provide crisis intervention and support to individuals and families in times of need.
7. Stay current on best practices and regulations in the field of social work.
8. Participate in departmental and organizational meetings and trainings.
9. Communicate effectively with clients, families, and healthcare team members.
10. Adhere to ethical and professional standards in all aspects of job responsibilities.
11. Continuously evaluate and monitor the effectiveness of interventions and make necessary adjustments to treatment plans.
12. Collaborate with community organizations and resources to provide additional support and services for clients.
13. Serve as a liaison between clients and their families and the healthcare system.
14. Provide education and support to clients and their families on coping skills and self-care strategies.
15. Maintain confidentiality and privacy of all client information.

The CHI St. Alexius Health regional healthcare system was formed in April 2016, when several Catholic Health Initiative healthcare facilities joined together to form the largest healthcare delivery system in central and western North Dakota.
