Social Worker

CHI St. Alexius Health is looking for a compassionate and dedicated Social Worker to join our team. Our ideal candidate will possess the necessary skills and qualifications to provide an excellent level of care and support.We are looking for someone with a Master’s Degree in Social Work from an accredited institution, a current license to practice social work in the state of North Dakota, at least one year of direct social work experience in a healthcare setting, and excellent communication and interpersonal skills. Responsibilities:

1. Provide social work services, resources, and support to patients and their families in a healthcare setting.
2. Develop and maintain relationships with patients and their families to ensure their safety and wellbeing.
3. Assess patient and family needs and provide appropriate interventions and referrals.
4. Utilize a strengths-based, trauma-informed approach to social work practice.
5. Collaborate with multidisciplinary teams to ensure the best possible care for patients.
6. Maintain accurate and up-to-date documentation on patient files.
7. Participate in professional development and continuing education opportunities.
8. Lead and facilitate group and individual counseling sessions.
9. Educate patients and families on available resources.
10. Advocate for patients and their families to ensure they receive the necessary care and support.
