• Dispensing and verifying the accuracy of medications for inpatient use
• Collaborating with healthcare professionals to develop and implement individualized treatment plans for patients
• Monitoring and assessing the safety and efficacy of medications for inpatients
• Providing drug information and education to patients, caregivers, and other healthcare professionals
• Reviewing and interpreting medication orders to ensure appropriate therapy and dosing
• Maintaining accurate and complete patient medication records
• Participating in multidisciplinary rounds and meetings to discuss patient care and medication-related issues
• Adhering to all legal and regulatory requirements for pharmacy practice
• Keeping up-to-date with new medications and treatment practices to provide the best possible care for patients
• Communicating effectively with patients, families, and healthcare team members to promote positive outcomes and patient satisfaction
• Collaborating with the pharmacy team to ensure efficient and timely delivery of medications to patients
• Identifying and reporting adverse drug reactions and medication errors to minimize patient harm
• Contributing to quality improvement initiatives and participating in medication use evaluations
