- Manage patient referrals: The primary responsibility of a Referral LPN is to coordinate and manage patient referrals within the healthcare system. This includes reviewing and processing referral requests, ensuring all necessary information is obtained, and communicating with various healthcare providers.
- Communicate effectively: The Referral LPN must possess strong communication skills to effectively communicate with patients, their families, and healthcare providers. This includes relaying information about referrals, scheduling appointments, and addressing any concerns or questions.
- Maintain organization: It is the responsibility of the Referral LPN to maintain organized and accurate records of all patient referrals. This includes keeping track of referral requests, documentation, and follow-up appointments.
- Provide exceptional patient care: As a member of the healthcare team, the Referral LPN plays a crucial role in ensuring patients receive the best possible care. This includes advocating for patients, addressing their needs and concerns, and ensuring a smooth referral process.
- Collaborate with team members: The Referral LPN must work closely with other healthcare professionals, including physicians, nurses, and administrative staff, to ensure a seamless referral process. This requires strong teamwork and collaboration skills.
- Stay up-to-date on healthcare policies and procedures: It is the responsibility of the Referral LPN to stay informed on current healthcare policies and procedures related to patient referrals. This includes staying up-to-date on insurance requirements, referral guidelines, and any changes within the healthcare system.
- Uphold confidentiality: As a healthcare professional, the Referral LPN must uphold patient confidentiality and adhere to all HIPAA regulations when handling sensitive patient information.
- Provide support to patients and families: Referral LPNs must provide support and guidance to patients and their families throughout the referral process. This may include explaining procedures, answering questions, and addressing concerns.
- Adhere to quality and safety standards: The Referral LPN must adhere to all quality and safety standards set by the healthcare organization. This includes following proper protocols
Valid Lpn License: The Candidate Must Hold A Current And Valid License As A Licensed Practical Nurse (Lpn) In The State Where The Job Is Located.
Experience In Healthcare Setting: Candidates Should Have At Least 1-2 Years Of Experience Working As An Lpn In A Healthcare Setting, Preferably In A Hospital Or Clinic.
Knowledge Of Catholic Health Care Values: Catholic Health Initiatives Is A Faith-Based Organization, And Candidates Should Have A Strong Understanding Of Catholic Health Care Values And Be Able To Integrate Them Into Their Practice.
Excellent Communication Skills: As A Referral Lpn, The Candidate Will Be Responsible For Communicating With Patients, Physicians, And Other Healthcare Professionals. Strong Verbal And Written Communication Skills Are Essential For Success In This Role.
Ability To Work In A Fast-Paced Environment: Catholic Health Initiatives Is A Large Healthcare Organization With A High Volume Of Patients And Referrals. The Candidate Must Be Able To Thrive In A Fast-Paced, Dynamic Environment And Be Able To Manage Multiple Tasks And Priorities Effectively.
